Dismissal Form

Early departure by a scout from a Cub Scout or Boy Scout program requires signed consent and compliance with dismissal procedures when an adult other than a parent will pick up and transport the scout , and for all other situations as described on the form.

MEDICAL FORM HAS 4 PARTS - A, B, C & D.

Use the Annual Health & Medical Record for:

  • SUMMER CAMP - USE ANNUAL FORM PARTS A, B & C PLUS APPENDIX FOR THE CAMP YOU WILL ATTEND for DAY CAMP, BEAR CLAW NYLT, CAMP STAFF, AND COUNSELOR-IN-TRAINING PROGRAMS -

  • OA FELLOWSHIP & SERVICE WEEKENDS and SAGAMORE SERVICE WEEKENDS, YOUTH USE PARTS A & B; ADULTS USE PARTS BASED ON LEVEL OF ACTIVITY.

  • UNIT WEEKEND CAMPING, CUB PARENT WEEKENDS, & WEBELOS WOODSUSE PARTS A & B FOR ALL YOUTH AND ADULTS

  • HIGH ADVENTURE ACTIVITIES - USE A, B, C, and D as directed by the activity base or director for the event. 

    Contact Philmont Scout Ranch, Florida Sea Base, or Northern Tier for the specific requirements by all youth and adults because of the strenuous nature of the activities taking place there.

    Unit leaders should have a complete medical history and permission slip for every participant attending every Scouting activity. The medical history form and permission slip, in most cases, will allow emergency medical treatment to a youth member in case of injury or illness when a parent or guardian cannot be contacted

2012 MEDICAL FORM APPENDIX FOR SUMMER PROGRAMS

The annual medical form must be accompanied by the medical form appendix for the camp attended.  The appendix is required to comply with NYS oversight of children's camps. These forms include:

  • the over the counter medication authorization to be signed by the parent and Scout's physician;

  • the medication forms that must be signed by a licensed physician  

 ONTEORA SCOUT RESERVATION Summer Programs

for Onteora Scout Reservation Home troop units, Council provisional troop, Counselor-in-training program, Bear Claw NYLT, Path to Eagle Camp and Camp staff.

SCOUTS WITH UNSIGNED OR INCOMPLETE FORMS WILL NOT BE ALLOWED TO STAY OVER AT CAMP.

A complete summer camp medical package will have the annual medical form plus these forms to comply with state regulations for children's camps.

A letter explaining the required signatures and a checklist is included.
